| DESCRIPTION | The theoretical teaching programme during the fifth year of studies is covered by a series of revision lectures, case-based teaching and tutorials delivered throughout the first semester. There is in addition a clinical component incorporating a three week clinical attachment. The student is required to keep a portfolio reflecting this clinical assignment. The assessment of the practical portfolio kept during the nine-week clinical assignment during the fourth and fifth year of studies will be incorporated in the final MDS4026 assessment. The study-unit will review the objectives set out in the previous study-units relevant to the specialty [MDS1009 - Reproduction & Genetics Units; and Obstetrics & Gynaecology covered in Year 4]. The scope of the study-unit is to train the student in translating theoretical knowledge into clinical practice. Study-unit Aims: The study-unit augments the clinical knowledge and skills learned in previous years, and prepares the student to have a comprehensive outlook to primary and preventive care for women’s health care needs. The principles of reproductive health care learned on in this study-unit will be integral in whatever area of medicine the individual will ultimately practice. The study-unit will train the student to address specific clinical problems of Reproductive Medicine; and will enforce relevant clinical skills. The study-unit will have Theoretical and Clinical components of learning and training. The theoretical component is aimed to cover the Core Learning Objectives aimed at training the students to deal with clinical problems from a practical viewpoint [problem-based learning]. The teaching component will include tutorials directed towards case-based learning. The Clinical component aims to further consolidate the student’s Core Clinical Skills and Competences related to women’s health care needs. The study unit will include an attachment with the clinical tutors of the Faculty. The student will be required to present a clinical log-book that includes case-summaries of a number of obstetric and gynaecological cases that the student has clerked and followed-up. Regular attendance to the clinical attachments, including emergency duties, is mandatory. Learning Outcomes: 1. Knowledge & Understanding: By the end of the study-unit the student will be able to: - Describe the principles and scope of vaginal examinations during labour and delivery. - Identify the possible obstetric complications that may arise in any of the trimesters of pregnancy. - Interpret the results of investigations [haematological, biochemical, physical] in the management of the normal or complicated pregnancy. - Explain to the patient the processes related to normal and abnormal labour and delivery. - Identify the problems that can occur during the postpartum period and puerperium. - Investigate the various menstrual-related abnormalities that may occur throughout her reproductive life. - Investigate the patient who presents with abdomino-pelvic pain. - Describe contraceptive measures. - Help the patient identify a suitable method of contraception. - Investigate and counsel the infertile couple. - Explain to the patient the various gynaecological operative procedures. - Investigate effectively the patient presenting with an abdomino- pelvic mass. - Identify and manage genital prolapse and urinary incontinence. 2. Skills: By the end of the study-unit the student will be able to: - Manage the routine care of a normal pregnancy in the clinical fields. - Identify the problem being presented by the patient. - Ask specific question to get an overall view of the clinical situation. - Fully assess clinically the obstetric and gynaecological patient - Establish the general health status of the patient. - Draw up a differential diagnosis based according to a working hypothesis based on the history and examination. - Make an investigation plan to further narrow the diagnosis options to initiate a treatment protocol. - Interpret the findings from the history and clinical examination within the obstetric and gynaecological setting. - Assess the obstetric patient fully throughout the whole of pregnancy and the puerperium, thus ensuring maternal and foetal well-being. - Safely manage the pregnant woman with the past or present medical or surgical complications. - Manage effectively the problems that can occur during the postpartum period and puerperium. - Evaluate and manage the various menstrual related abnormalities that may occur in the female throughout her reproductive life. - Assess and manage the gynaecological patient who presents with abdomino-pelvic pain. - Manage the patients found to have an abnormal cervical smear. - Manage and treat patients with a genital infection. - Initiate a management plan. - Explain various aspects of the various obstetrics and gynaecological procedures to the patient.
