DESCRIPTION | The unit is intended to provide the student with an immersion in the real practice. The student will undertake two experiences, one in each semester of followed activities that draw on practical scenarios. Each experience will consist of 7 seminars of 3 hours each and 10 sessions of 3 hours in a practical scenario namely a clinical pharmacy setting in a hospital or community pharmacy. The experiences will be scheduled after each of the clinical rotations. Students will be required to complete a workbook and present reflections on the experience. Study-unit Aims: - To present a hands-on experience of processes in patient management namely in patient medication history taking, identification of pharmaceutical care issues, pharmaceutical care plan development, pharmacotherapy management, patient monitoring, medication reconciliation activities, patient counselling; - To experience development and application of medication assessment tools and audit exercises; - To develop an approach for reflective recommendations in therapy optimization. Learning Outcomes: 1. Knowledge & Understanding: By the end of the study-unit the student will be able to appreciate use of guidelines, comparison of drug use and critical appraisals. 2. Skills: By the end of the study-unit the student will be able to: - Develop a teamwork approach; - Develop Medication Assessment Tools which have a practical application within specific scenarios; - Assess the quality of prescribing in defined therapeutic patient populations; - Implement evidence-based results to individualise and optimise pharmacotherapeutic decisions with a patient focus; - Identify patients requiring advanced pharmacist interventions.
