Abstract: A 59-year-old man presented with a 3-week history of fever, episodic vomiting, epigastric discomfort, loss of appetite, and weight loss. His medical history included immunoglobulin A deficiency, chemotherapy, and radiotherapy for non-Hodgkin’s lymphoma and splenectomy. He was febrile, had epigastric tenderness, and a 6-cm hepatomegaly (previously present and investigated). Blood tests revealed a normochromic normocytic anemia and increased inflammatory markers. Radiologic studies only revealed hepatomegaly (no increase in size). A biopsy examination of the hepatomegaly showed sinusoidal dilatation (previously present in 1991). Human immunodeficiency virus testing was negative.
