The condition of acute polyneuropathy has intrigued clinicians especially since the demonstration by Guillain, and Strohl in 1916 of the unusual discrepancy between the high protein content of the cerebrospinal fluid and the normal cellular count. The course of the illness can be fulminating and the differential diagnosis may be difficult. An illustrative case is described here, whereby a three year old boy had been apparently healthy till he complained, two days before admission, of pain in the calves which he claimed was relieved if he stood on his toes. Certain aspects of the disease are discussed with special reference to the difficulties in differential diagnosis. One feature that is worth stressing is the frequent late appearance in the course of the disease of the protein rise characteristically often seen in this condition.en_GB
