Abstract: Nursing documentation is a tool that helps health care professionals to share information about the patients they care for and also helps to enhance better continuity of care (Hager & Munden, 2008). In addition, documentations makes nurses visible to other professions, because it helps to give a ‘voice’ to the nursing profession (Hyde et al. 2004) The objectives of the study were as follows: 1. To explore the views of nurses regarding the use of documentation in primary health care 2. To explore their practice towards documentation 3. To explore factors that encourage proper documentation practice 4. To explore factors that hinder proper documentation practice.
