Abstract: A 27-year-old left-handed man sustained an injury to his dominant middle finger from airbag deployment during a low speed road traffic accident. On examination the finger was swollen, with a mallet finger deformity. Radiographs confirmed an associated dorsal fracture fragment with no palmar subluxation of the distal interphalangeal joint (Fig 1). The finger was placed in a mallet finger splint for 6 weeks, at which stage it was found that there was no active flexion of the distal interphalangeal joint. This aroused clinical suspicion of a ruptured flexor digitorum profundus tendon and this diagnosis was confirmed by ultrasonography.
